ADHD Assessment for Kids and Teens Ages 4-17
The process of getting your child evaluated for ADHD at Lily Pad Mental Health:
1. The first step is the same for all clients: the intake assessment. This is the first full appointment where we discuss presenting concerns for therapy, family history, current symptoms, goals, etc.
2. The next step is the clinical interview. We talk more in depth about ADHD concerns, your child’s behaviors, areas of life being affected, and how these behaviors show up in the day-to-day with your child.
3. I will provide you with questionnaires for you to complete at home. These will assess for ADHD as well as other potential mental health concerns. I will provide your child’s teacher with several questionnaires to complete as well. Depending on your child’s age, there may be self-report questionnaires for them to complete.
4. After we have done the clinical interview and once ALL forms and questionnaires have been completed and returned to me, I will begin scoring them and I will write a report detailing the results, any appropriate diagnoses, as well as recommendations.
5. Reports are typically completed within 1-2 weeks from the time that the last form/questionnaire has been returned to me. I will contact you to let you know the report has been completed and I will upload it to your client portal.
The cost of the ADHD evaluation:
$140 Standard Intake Assessment (for current clients, this will have already been completed)
$375 Clinical Interview, ADHD Evaluation Scoring and Written Report
Total for new clients: $515
Total for current clients (who have already completed the intake assessment): $375
It’s true that getting your child evaluated for ADHD is a process. But for you to be able to better understand your child, identify the “why” when it comes to behaviors that have been difficult to manage, and to empower both you and your child so that they can receive the support and resources that will help them thrive, it’s well-worth the time and investment.

Benefits of getting your child evaluated for ADHD:
So that you and your child can understand why they are struggling in certain areas of their life.
To be able to advocate for your child in school and for them to advocate for themselves as they get older. This is important if you are seeking a 504 plan, an IEP, or any other support at school.
To be able to provide this information to their doctor if you are interested in exploring ADHD medication as an option.
To build and increase empathy and understanding between you and your child.
So that you can set realistic expectations for your child once you understand the areas where they are struggling due to ADHD. It’s important to know what is and isn’t in their control and how to support them.
So that your child can understand that there is a reason why they have a hard time with certain tasks or situations and they are not “broken.”
By getting your child evaluated before adulthood, you can help reduce the likelihood of them developing depression and anxiety over time, which often occurs as a result of having undiagnosed ADHD.
